Where they differ

La Spezia is the one for time in the water: 32 beaches nearby against Milan's 4, and four months with the sea at 21°C or above, which Milan cannot match at all. Its few bars sit close to the centre, so evenings are walkable if short.

Milan has many times as many notable places within walking distance of its centre, so days need less transport. For children it has far more attractions and park space near the middle, and 5 theme parks within reach against La Spezia's 2.
