The verdict
Choose Milan if you want the wider choice of restaurants and bars, more to keep children occupied, and protected country and parkland within reach for days out of town.
Choose Naples if the sea is the point: 29 beaches nearby, water warm enough to swim in from June to October, and a compact centre of notable places to cover on foot.

Where they differ
Milan has about twice as many restaurants near the centre, a much wider range of kitchens, and about twice as many places to drink. For families, it has many times as many attractions near the centre, twice as much park space, and 5 theme parks within reach against 3. Around Milan, 30% of the land is protected, against 7% around Naples.
Naples has the sea: 29 beaches nearby against Milan's 4, and 5 months of water at 21°C or more, where Milan has none.
Where they are alike
Both carry a comparable weight of museums, and both sit within day-trip reach of UNESCO-listed places, 5 from Naples and 4 from Milan. Either will fill several days of indoor heritage without repetition.
Their centres work in a similar way: notable places packed into walking distance, with Naples holding somewhat more. Both have drinking and clubbing, though Milan's is larger and half of it lies away from the centre, while in Naples most of it is central.
Which traveller are you?
Travelling with children, Milan is the clearer choice: many times as many attractions near the centre, twice as much park space, and 5 theme parks within reach against 3.
If the point of the trip is swimming, only Naples delivers: 29 beaches nearby and water at 21°C or more for 5 months, with the heritage of a dense centre a short move inland.
For eating and going out, Milan has roughly twice as many restaurants near the centre and a much wider range of kitchens, plus about twice as many bars and clubs, though they sit spread out rather than clustered centrally.
What the numbers don't tell you
The value comparison between these two carries no information. Both are placed on the same national price level, so it is identical by construction, and the €155 a day beside it is one national estimate rather than a measurement of Milan or Naples. Budget on the trip you plan, not on that figure.
Naples's beach advantage runs to only 5 months of swimmable sea, and those are the same weeks everyone else picks. Milan's nature strength is measured on the country around the city, 30% of it protected, so it means days out and transport rather than green space at the end of a street.
When should you go?
The figures behind each month. Each row is shaded against that destination's own year, so the deepest cell is its best month for that measure and the palest its weakest. Shades are not comparable between the two rows.
Temperature and rainfall are thirty-year climate normals, averaged over the years shown. Naples: 1991-2020 normals, Grazzanise (28.3 km from centre). Milan: 1991-2020 normals, Novara / Cameri (41.2 km from centre).
Milan is at its best from June to August, with highs of 26-29°C and the year's lightest rain. September to November are the wettest stretch, above 110mm a month, and winter days are cold.
Naples holds up longer. July and August are hot at 31°C with almost no rain, 17-18mm, and the sea peaks at 26°C. October still reaches 23°C by day. November is the wettest month either city sees, 159mm.

The busiest departure cities for these two, with a place kept for each other part of the world. Busiest means arriving passengers where an official count exists, and where none does, the number of non-stop services flown: official route statistics are published by the European reporting countries and by nobody else, so a table ranked on them alone would offer a reader in New York or Tokyo ten European cities. Flying times are estimated from the distance between airports, and every row is a city flown non-stop to both. Where the typical offer connects anyway, the stops are shown. Under each figure is the airport the flight lands at and how far that is from the centre, because the nearest airport is not always the one a route is flown to. Where both columns name the same airport it is one flight, and the difference between the two is the distance printed beside it. Typical return fares for 2026-09 to 2026-10, cached market data up to 7 days old. The figure is the middle half of the offers found, so a single price means they agreed and a range means they did not; for a typical route that middle half spans about a quarter of the median, and for the least settled routes rather more than the median itself.
